CLINICAL AND HISTOPATHOLOGICAL RESULTS OF PATIENTS WITH SQUAMOUS CELL CARCINOMA OF THE LOWER LIP WITH NECK DISSECTION

Objective: We aimed to discuss along with current literature the clinical findings and histopathological results of surgical materials of patients with squamous cell carcinoma in the lower lip and undergoing neck dissection.

Materials and Methods: Medical records of 41 patients who had squamous cell carcinoma in the lower lip and were performed surgical treatment and neck dissection between 2015 and 2022 were investigated retrospectively. Demographic features of the patients, tumor site (right, left or midline), smoking and alcohol consumption history were reviewed. The largest diameter of the tumor, tumor depth and T stage were found from chart records. Perineural and lymphovascular invasion, neck metastasis and extracapsular spread were investigated from histopathological results.

Results: There were 33 male (%80.5) and eight female (%19.5) patients in this study. The ratio of male to female was 4,1:1. The mean age of the patients was 60,41±14,17 years (range 33-86 years). When T stages were examined, T1 in 24 patients (58.5%), , T2 in 12 patients (29.3%) tumors, T3 in one patient (2.4%), and T4 tumor in four patients (9.8%) were detected. Thus, 87.8% of the patients had the early stage tumor (T1 and T2) and 12.2% had the advanced stage tumor (T3 and T4). Neck metastasis was seen in patients with advanced stage tumor.

Conclusion: Lip cancers could be detected at an early stage and they are seen more commonly in male patients and advanced age. In squamous cell carcinomas of the lower lip, metastasis to lymph nodes of the neck is more common in advanced stage tumors. While excising tumor from the lower lip, a safe surgical margin, oral competence and aesthetic concerns should be taken into consideration.
