EVALUATION OF OSSICULAR PATHOLOGY IN CHRONIC OTITIS MEDIA WITH PREOPERATIVE AUDIOLOGY
1Karabük Üniversitesi Karabük Tıp Fakültesi, Kulak Burun Boğaz Anabilim Dalı, Karabük, Türkiye Aims: Chronic Otitis Media (COM) is an inflammatory disease characterized by tympanic membrane perforation, ear discharge, and hearing loss. Although preoperative audiology provides information about the middle ear for COM surgery, sometime preoperative audiology and preoperative findings are in conflict. The aim of this study was to determine the effect of hearing on the ossicular pathologies in the middle ear. For this purpose, preoperative audiologic findings were compared with those of peropepertive ossicular pathology.Methods: 60 patients with COM and 30 normal individual were included in this study. According to peroperative ossicular pathologies, 42 (70%) of the cases had ossicular pathology.18 (30%) had only tympanic membrane perforation. Incus erosion was seen in 36 (60%) cases, malleus erosion in 33 (55%) and stapes erosion in 12 (20%) cases. Patients were divided into 5 groups according to their ossicular pathologies.
Results: Results of Group1 hearing levels were 27.83 dB, Group 2: 56.50 dB, Group3: 71.67 dB, Group 4: 67.80 dB, Group 5: 78.75. The mean hearing levels of the control group were 11.83 dB. The mean hearing loss was the highest in group 3,4,5 and was statistically significant compared to the control group and all other groups (p <0.05). Group 5 was the highest in numbers but there was no significant difference between 3,4 (p> 0.05).
Conclusion: Hearing loss in chronic otitis media is due to ossicular lesions rather than tympanic membrane perforation. Hearing loss within 45-50 dB suggests ossicular pathology. Although preoperative high hearing loss in chronic otitis media shows ossicular damage, it does not always indicate the severity and destruction of the disease.
