Papillary carcinoma occuring in a thyroglossal duct cyst is rare. We herein present two cases who underwent Sistrunk procedure for throglossal duct cyst and pathologic examination revealed papillary carcinoma. There was no capsular invasion and the surgical margins were negative. Postoperative thyroid scintigraphies were normal. Computed tomography examinations displayed no cervical lymphadenopathy. The patients have been followed up for two and five years with no further evidence of disease. For papillary thyroid carcinoma arising in a thyroglossal duct cyst, the Sistrunk procedure is considered adequate initial treatment. Further surgery depends on the finding of thyroid nodules or positive lymph nodes. Postoperative radioactive iodine ablation may be considered in cases of recurrence or metastasis.
