RESULTS OF PALISADE SEPTOPLASTY METHOD IN SADDLE NOSE DEFORMITY REPAIR
1Sağlık Bilimleri Üniversitesi, Samsun Eğitim ve Araştırma Hastanesi, Kulak Burun Boğaz Servisi, Samsun, Türkiye Objective: Saddle nose deformity is a disease that can be caused by septum trauma, surgery, inflammatory diseases, tumors and intranasal drug use. The aim of the treatment is to regenerate the missing tissue and restore the respiratory physiology of the nose. The purpose of our new septum modeling is to obtain the closest septum to the natural septum anatomy and to evaluate the results of the new septum model.Material and Methods: 9 patients who underwent palisade technique due to advanced saddle nose deformity were included in the study. 3 of the patients were female and 6 were male and their average age was 35.2 (26-48) years. 3 of the patients had a previous rhinoplasty operation, 5 patients had a history of nasal trauma in childhood, and 1 patient had no information about the cause in the anamnesis. Patients were approached using the open rhinoplasty method. In this study, a costal cartilage was used to create the septum, and a new septum was created by preparing the strips prepared from the costal cartilage in the form of a palisade. The dorsal area of the newly formed septum was supported with two cartilages in the form of spreader grafts. The Palisade-shaped septum was placed in its new place and fixed with sutures to the mucosa in the crest region at the base, the medial cruses of the alar cartilage at the front and the perpedikular area at the back. The results of the patients after 1 year were evaluated.
Results: A year after the operation, there were no functional and cosmetic problems. During the control period, there were no complaints of bending and collapsing of the nose that would require revision surgery. Based on the anamnesis of the patients, they reported that their breathing was better than before the surgery.
Conclusion: This new method can be safely applied to create the septum in severe saddle nose deformities.
