MANAGEMENT OF CHILDREN WITH ANRTROCHOANAL POLYP

3Başkent Üniversitesi Tıp Fakültesi, KBB Anabilim Dalı, Ankara, Türkiye Objectives: Antrochoanal polyps, particularly in children, have a tendency for recurrence when they are insufficiently treated and they increase the incidence of intractable chronic sinusitis. In all nasal polips, antrochoanal polips are observed in children five to eight times as frequently as in general population. Recently, the endoscopic sinus surgery has been the most remarkable approach in the treatment. That transcanine sinuscopy has been added in selected cases is important to prevent recurrence. Investigating allergic reasons in etiology may contribute to further medical theraphy.
Case report: The patient given consultation by the department of pediatrics with complaints of nasal obstruction and cough for 5 months was a 9-year-old boy. There existed a solid polypoid mass which had fully filled the right nasal passage at anterior rhinoscopy. The view of the CT of paranasal sinuses was in accordance with antrochoanal polyp. Simple excision of intranasal part, limited uncinectomy, the maxillary sinus ostia widening and transcanine sinuscopy procedure were applied and all pathology was cleaned. Meanwhile, the eosinophilia on the nasal smear was viewed and total and specific immunoglobulin E tests and prick tests were carried out.
Conclusion: We discussed the related therapeutic approaches in view of the case of that child who had developed antrochoanal polyp and who we had managed through minimally invasive surgery. Limits of endoscopic sinus surgery and importance of transcanin sinuscopy were emphasized in the child with antrochoanal polyp. Contribution of allergy to etiopathogenesis and its effects on medical treatment were mentioned.
