MANAGEMENT OF PAPILLARY THYROID CARCINOMA ASSOCIATED WITH SQUAMOUS CELL CARCINOMA OF LARYNX
1Hacettepe Üniversitesi, Kulak Burun Boğaz Hastalıkları, Ankara, Türkiye2Hacettepe Üniversitesi, Patoloji, Ankara, Türkiye Objectives: To report three patients with thyroid cancer synchronously found in the specimen of laryngectomy for laryngeal squmous cell cancer and to evaluate the treatment approach.
Study Design: Retrospective case series.
Subjects and Methods: Patients with laryngeal cancer treated in Department of Otorhinolaryngology Head Neck Surgery Hacettepe University Faculty of Medicine between 2005 and 2011 were enrolled. There were 112 patients treated with surgery for laryngeal cancer.
Results: 33 patients underwent partial laryngectomy, 79 patients underwent total laryngectomy. Simultaneous neck dissection was performed to 85 patients. There were thyroid cancer foci in thyroid tissue of total laryngectomy specimen in two patients. One has had a total thyroidectomy during laryngectomy, but second patient underwent hemithyroidectomy during surgery, did not have any completion thyrodiectomy. In the third patient, who had partial laryngectomy and neck dissection, there were thyroid cancer metastases in two lymph nodes in histopathologic examination of the neck. He had subsequent thyroidectomy after the initial operation. All patients were followed up.
Conclusions: It is rare to discover an unsuspected simultaneous thyroid cancer in the thyroid tissue removed in laryngectomy. A conservative follow-up approach is suggested in advanced layngeal cancer, however, subsequent total thyroidectomy is required for early stages.
