Sphenoid sinus mucocele is a rare, but well-known clinical entity. Symptoms usually arise from the compression of neighbouring structures. We describe a case of sphenoid sinus mucocele presenting with abducens nerve palsy. Emergent endoscopic sinus surgery with marsupialization of the mucocele resulted in immediate resolution of symptoms. Sphenoid sinus mucoceles may result in various symptoms due to its proximity to many vital anatomical structures. They should be kept in mind in the differential diagnosis of abducens nerve palsy and early surgical intervention is curative in such a condition.
