Choanal atresia is a rare cause of developmental obstruction of the posterior nasal opening. The clinical presentation varies from immediate postnatal cyanosis and respiratory distress to nasal obstruction with an associated persistent mucoid discharge. The latter symptoms are nonspesific and are shared by a host of other nasal disorders as rhinitis, sinusitis. Thus, unilateral choanal atresia often eludes diagnosis and correction is delayed until adulthood. The transpalatal surgical approach is the most popular method. In this article we describe a transpalatal repair of an unilateral choanal atresia with isolated sphenoid sinus pathology in an adult patient.
