CHRONIC INFLAMATORY MAKSILLARY SINUS PATHOLOGIES: IS INTERVENTION TO MIDDLE MEATUS BY ENDOSCOPIC ENDONASAL ROUTE ADEQUATE EXACTLY?

Objective: To find out if endoscopic endonasal intervention to the chronic inflamatory pathologies of the middle meatus which are suspected as major factors in the maxillary sinus retention cysts and marked chronic inflammation is adequate or not.

Study Design and Setting: Prospective, randomized clinical study in a tertiary setting

Methods: Study group was composed of 100 patients who had maxillary retention cysts or marked chronic inflammation besides chronic inflammation of the osteomeatal unit and one or more sinuses. The study group was separated into two subgroups equally and randomly. Only endoscopic endonasal middle meatus intervention was performed for the first subgroup and anterior antral route and clearence of the pathologies in the maxillary antrum under direct visualisation were added in the second subgroup. Subjective comparison was performed with SF-36 and TyPE Spesific surveys and objective comparison was performed by means of Lund-Mackay staging system.

Statistics: Paired t test for the results of the same subgroup before and after operation and ANOVA test for the different groups were used.

Results: The surgical treatment was prooved useful for both groups. Maxillary Sinus Scores and TyPE Spesific Surveys result were better in the Modified Caldwell-Luc subgroup.

Conclusion: Addition of the Modified Caldwell-Luc procedure to the endoscopic endonasal middle meatus intervention may results better surgical outcomes in patients with maxillary sinus retention cysts and marked chronic inflammation.
