OUR ENDOSCOPIC TYMPANOPLASTY RESULTS IN LARGE PERFORATIONS

Objective: Tympanoplasty is a surgical technique that is used in chronic otitis media surgery. The main purpose of the endoscopic tympanoplasty is to create a solid, dry eardrum with graft material that closes the perforation area, prevents chronic otitis media sequelae and provides auditory recovery. In this study, our aim is to evaluate our patients who had tympanoplasty using endoscopic transcanal chondro-perichondrial graft and who had large perforation of the tympanic membrane due to chronic otitis media.

Material and Methods: 38 patients who had surgery, endoscopic tympanoplasty January 1, 2019 -January 1, 2021 were retrospectively included in the study. Patients with >5 mm central perforation, had preoperative pure tone audiometry, and no ear disease for at least 3 months, who had endoscopic type 1 tympanoplasty surgery were included in the study.

Results: The mean age of 38 patients who had endoscopic tympanoplasty was 23.55 ± 10.05 years and the female-male ratio was calculated as 21:17. The mean operation time was 44.05 ± 5.57 minutes. In 35 of the patients, the graft was intact at the 6th month postoperatively (92.1%). In the postoperative 6th month hearing test measurements, postoperative air-bone gap values and statistically positive improvement were observed compared to the preoperative tests.(p<0.001).

Conclusion: Endoscopic type 1 tympanoplasty results in a high graft success rate and improved hearing results, regardless of the location and size of the perforations. Endoscopic tympanoplasty helps in providing high patient safety and comfort in chronic otitis media surgery. Keywords : Endoscopic tympanoplasty; central large perforation; cartilage
