A SALVAGE TREATMENT IN PATIENTS WITH SUDDEN HEARING LOSS: INTRATYMPANIC STEROID

Objective: To analyse patients who were treated with intratympanic 4mg/ml dexamethasone injection after failure of systemic steroid treatment.

Methods: Fifty patients(24 female,26 male) with sudden sensorineural hearing loss and had salvage treatment(intratympanic dexamethasone) between 2008 to 2015 were included. Efficacy of treatment was evaluated by average low frequency level(250,500,1000,2000 Hz), average high frequency level(4000,6000,8000 Hz) and speech discrimination score.

Results: The difference between pre-treatment (68.99 ±30.71) and post-treatment (62.54±33.81) high frequency average values was statistically significant(p<0.05). The difference between pre-treatment(68.17 ±28.41) and post-treatment (56.95 ±30.56) low frequency average values was statistically significant(p<0.05). The difference between systemic steroid treatment and intratympanic steroid injection low frequency average values was statistically significant(p<0.05). The difference between pre-treatment(55.94 ±32.9) and post-treatment (62.45 ±31.21) speech discrimination scoreswere statistically significant(p<0.05).There was statistically significant difference between patients who had 4 or fewer intratympanic injections and 8 or more injections(p<0.05).

Conclusion: Our study showed thatintratympanic steroid treatment after failure of systemic steroid is effective in all frequencies. Keywords : Intratympanic steroid, sudden hearing loss, salvage treatment
