IS IT NECESSARY TO MAKE A THYROIDECTOMY IN ADVANCED STAGE LARYNX CANCERS?

1Dicle Üniversitesi, Kulak Burun Boğaz ve Baş Boyun Cerrahisi Anabilim Dalı, Diyarbakır, Türkiye Introduction: The aim of this study is to determine the thyroid gland invasion in advanced laryngeal cancers and to investigate whether there is a relationship between thyroid gland invasion and the anatomical region where the tumor is located, T staging of the tumor and extralaryngeal spread.

Material method: We retrospectively reviewed patients who underwent Total Laryngectomy for advanced laryngeal cancer (T3 and T4a) in the Otorhinolaryngology Clinic of Dicle University between 2010 and 2018. The patients were divided into two groups as patients with thyroid gland involvement (TGI +) and patients without thyroid gland involvement (TGI -). In addition, the presence of thyroid gland invasion, subglottic region invasion, invasion of the thyroid and cricoid cartilages, prelaryngeal lymph node positivity, tumor size as a result of pathological examination, and the presence of angiolymphatic and perineural invasion were noted. A comparison was made between the two groups according to these findings.

Results: No statistically significant difference was found in patients with thyroid gland invasion in terms of age, gender, clinical stage, lymph node involvement, tumor size, perineural invasion rate and lymphovascular invasion rate. Cricoid cartilage invasion rate, subglottic region involvement rate, thyroid cartilage invasion rate and cricoid cartilage invasion rate were found to be significantly higher in patients with thyroid gland invasion.

Conclusion: Cricoid cartilage involvement, thyroid cartilage involvement and subglottic invasion are risk factors for thyroid gland invasion. In total laryngectomies, it is necessary to decide on thyroidectomy according to these risk factors and unnecessary thyroidectomy should be avoided.
