OUR INDICATIONS FOR EXTERNAL FRONTAL SINUS SURGERY IN THE LAST 10 YEARS

Sağlık Bilimleri Üniversitesi Haseki Eğitim ve Araştırma Hastanesi, Kulak Burun Boğaz Hastalıkları Kliniği, İstanbul, Türkiye Objective: Endoscopic sinus surgery has been progressing for the last 30 years to begin to be used for treatment for a variety of sinus disorders. Indications for external sinus surgery have decreased in the meantime. We aimed the investigate the areas of use of external frontal sinus surgery in our time.

Methods: Files of patients who underwent external frontal sinus surgery between January 2008 and December 2017 in our department were examined retrospectively. Age, gender, indication for surgery and type of surgery were noted. Indications for surgery and numbers of surgery per year were also investigated.

Results: There were 18 cases who underwent external frontal sinus surgery in the last 10 years. The age was 42±20(14-84) years. The most common indication was frontal sinus osteoma(38.8%) followed by mucocele (27.7%). Infectious indications such as osteomyelitis(16.5%), intracranial complication of frontal sinusitis (5.5%), and neoplasia such as inverted papilloma(5.5%) and hemangiopericytoma(5.5%) were the other indications. The most common type of surgery was osteoplastic flap(72.2%) followed by trephination(22.22%) which was performed most recently 7 years ago. The frequency of surgery was 0-4 cases/year. 61.11% of cases underwent surgery combined with intranasal endoscopic surgery.

Conclusion: Unability to reach with endoscopy, previous failure with endoscopy and distorted anatomy due to previous surgery may necessitate external approaches. External frontal sinus surgery is still an important therapeutic modality. Keywords : External frontal sinus surgery, osteoplastic flap, trephination
