Fungal colonization is a rare condition in paranasal sinuses. In this case, we present computed tomography findings of fungal sinusitis in a patient who had chronic liver disease and was a candidate for liver transplantation. Preoperative diagnosis of fungal sinusitis is very important in patients who will undergo transplantation, because, risk of fungal sinusitis is high in these patients with immunsuppression. Unenhanced computed tomography imaging of paranasal sinuses provides valuable information for initial diagnosis of fungal sinusitis, treatment planning and evaluating the course of disease. Fungal sinusitis should be considered in the differential diagnosis, in the presence of hyperdense areas with irregular margins within the paranasal sinuses on unenhanced computed tomography in patients who are immunsuppressed or have chronic systemic diseases as in our case.
